Job Description
Remote Medical Assistant for Chronic Care Management specializing in high-utilizing Medicaid patients. Requires living in CA and collaborating with providers for patient care plans.
Responsibilities:
- Visit notes & Data collection
- Taking patient phone calls
- Looking at lab values & Imaging
- Creating care plans in collaboration with CCM and Provider
- Handling 90% of the admin work for the CCM provider
- Specialty service line participation
Requirements:
- California residents highly preferred
- Bachelors Degree Required
- Current Medical Assistant certification
- 3 years experience as a medical assistant (Preferred)
- Bilingual (preferred)
