Job Description
Analyst, Outreach Coordinator improving healthcare outcomes by addressing gaps in care at CVS Health. Engage with providers and members through data analysis and effective communication.
Responsibilities:
- Utilize several tools to help close care gaps throughout the year
- Facilitate meetings with providers and internal partners to address gaps in care
- Answer questions regarding all facets of the HEDIS® process
- Be the subject matter expert regarding HEDIS measures and help devise a plan to assist members
- Support provider engagement and actively participate in workgroups
- Deliver quality information to internal and external constituents via written and verbal communication
- Seek opportunities for process improvement
Requirements:
- 3+ years' of recent and related health care experience in any setting
- 1+ years member/provider education experience targeting gaps in care
- Experience successfully performing in an environment where daily/weekly productivity metrics are expected
- Ability to work independently
- Ability to use standard corporate software packages and applications
- Strong communication skills
