Job Description

Analyst, Outreach Coordinator improving healthcare outcomes by addressing gaps in care at CVS Health. Engage with providers and members through data analysis and effective communication.

Responsibilities:

  • Utilize several tools to help close care gaps throughout the year
  • Facilitate meetings with providers and internal partners to address gaps in care
  • Answer questions regarding all facets of the HEDIS® process
  • Be the subject matter expert regarding HEDIS measures and help devise a plan to assist members
  • Support provider engagement and actively participate in workgroups
  • Deliver quality information to internal and external constituents via written and verbal communication
  • Seek opportunities for process improvement

Requirements:

  • 3+ years' of recent and related health care experience in any setting
  • 1+ years member/provider education experience targeting gaps in care
  • Experience successfully performing in an environment where daily/weekly productivity metrics are expected
  • Ability to work independently
  • Ability to use standard corporate software packages and applications
  • Strong communication skills
