Job Description
Care Connector resolving complex patient and member concerns for Geisinger’s Pennsylvania healthcare system. Providing clinical support, crisis triage, benefits guidance, and resource coordination from home.
Responsibilities:
- Assist in resolving patient/member concerns and provide clinical education and instruction
- Maintain patient/member satisfaction through a high level of clinical support
- Assist and educate patients/members through motivational interviewing, problem solving, interpreting complex clinical problems, researching information, contacting providers and providing information
- Help patients/members address social determinants of health, special needs and crisis situations
- Identify primary and secondary resources, discuss options and help patients/members decide on a plan of action
- Advocate for incapacitated patients/members or those unable to communicate effectively with service delivery systems
- Serve as primary contact for the Enhanced Member Supports Unit, Behavioral Health and network for patients/members/providers
- Assess clinical issues and concerns and provide accurate information promptly
- Manage crisis situations and triage appropriately
- Complete resolution of patient/member/provider concerns and inquiries
- Provide daily support and direction to patient/member/provider services staff
- Obtain caller-needs data for monthly statistical and activity reports
- Communicate and coordinate with internal departments to resolve issues
- Collect and record data on special-needs organizations, facilities and services
- Prepare, generate and disseminate letter communications to patients and members
- Provide team training as needed
- Explore covered benefits and community resources by line of business and product design
- Provide team-building resources and direct peers to appropriate policies and procedures
- Complete outbound calls to resolve complex eligibility issues
- Navigate multiple computer programs while assisting patients/members/providers
Requirements:
- Bachelor's Degree in a related field of study (Social Work, Healthcare Administration, Psychology, Public Health, Human Services, or a related healthcare/behavioral health field)
- Minimum of 1 year of related work experience
- Must live in Pennsylvania
- Must be available for 40 scheduled weekly hours, 8:30am–5pm day shift
- Does not qualify for J-1 waiver
