Job Description
Clinical Informatics Registered Nurse providing telephonic triage and chronic disease management to patients. Collaborating with care teams to ensure optimal patient outcomes in a remote setting.
Responsibilities:
- Provides telephonic triage to all incoming patient/caregiver calls as assigned
- Direct interaction, education, and follow-up with patients and caregivers
- Guides patients in chronic disease management concepts
- Acts as a patient advocate in coordinating care
- Documents all patient/staff interactions
- Contacts physicians with updates of patient status
Requirements:
- Licensed RN with current state license
- Excellent telephone and communication skills required
- 2-3 years of clinical practice in their respective field
- 1-3 years of home health/hospice experience
- Knowledge of disease management and chronic disease processes
- Prior management experience preferred
- Ability to work independently and within a multidisciplinary team.
