Job Description
Remote endocrinology physician providing obesity and complex diabetes care for Form Health’s virtual clinic. Managing telemedicine patients, CGMs, insulin pumps, and multidisciplinary APP collaboration.
Responsibilities:
- Provide direct patient care in a telemedicine setting for obesity and comprehensive diabetes management
- Manage insulin titration and patients using insulin pumps and continuous glucose monitors (CGMs)
- Maintain an individual patient panel or collaborate with and supervise Endocrine APPs and Registered Dietitians
- Review charts and conduct initial and return visits
- Manage CKD/ESRD and CAD/CV conditions
- Provide advanced treatment planning and medical management for complex endocrinology cases
- Prescribe non-controlled obesity and diabetes medications
- Ensure clinical documentation, protocols, and treatment pathways are followed
- Support colleagues’ professional development, clinical growth, feedback, and continuing education
- Participate in team-based care initiatives and quality improvement efforts
- Attend departmental meetings
Requirements:
- Specialized in Endocrinology and Obesity Medicine
- Valid medical license to practice and prescribe legally in any US state
- Eligibility to obtain additional state licenses as needed
- American Board of Internal Medicine (ABIM) certified Endocrinologist
- MD or DO degree
- Valid and current Board certification in Endocrinology by the American Board of Internal Medicine (ABIM)
- 2 years of clinical experience and pharmacotherapy in obesity medicine or endocrinology, post residency, OR completed an accredited Fellowship program in Endocrinology or Obesity Medicine in lieu of experience
- Active and unrestricted Medical State License in any US State
- Eligibility to obtain additional state licenses; IMLC preferred but not required
- Currently opted-in with Medicare
- Competency managing CKD, ESRD, and CAD/CV conditions
- Expertise in CGMs, insulin pumps, and complex insulin regimens
- Experience working with APPs, medical residents, or trainees in a clinical academic setting
- Adherence to evidence-based treatments, behavioral interventions, nutrition, and pharmacotherapy for endocrine disorders and obesity
- Open to working one of the following W2 roles: 32hrs/week, 36hrs/week or 40hrs/week
- Experience providing synchronous patient care in a telehealth setting is preferred
- Bilingual candidates with medical fluency in Spanish are desired, but not required
