Job Description
Endocrinologist providing remote obesity and diabetes care through Form Health’s multidisciplinary telemedicine clinic. Managing complex diabetic comorbidities, CGMs, insulin pumps, and APP collaboration.
Responsibilities:
- Provide direct patient care through telemedicine for obesity and comprehensive diabetes management
- Perform insulin titration and manage patients using insulin pumps and continuous glucose monitors
- Maintain a patient panel or collaborate with and supervise Endocrine APPs and Registered Dietitians as needed
- Review charts and conduct initial and return visits
- Manage CKD/ESRD and CAD/CV conditions
- Provide advanced treatment planning and medical management for complex endocrinology cases
- Prescribe non-controlled obesity and diabetes medications
- Ensure consistent clinical documentation, protocols, and treatment pathways
- Foster colleagues' clinical growth and support continuing education
- Participate in team-based care, departmental meetings, and quality improvement initiatives
Requirements:
- MD or DO degree
- Current ABIM Board certification in Endocrinology
- 2 years of clinical experience and pharmacotherapy in obesity medicine or endocrinology post residency, OR completion of an accredited Fellowship program in Endocrinology or Obesity Medicine in lieu of years of experience
- Active and unrestricted medical license in any US state
- Eligibility to obtain additional state licenses; IMLC preferred but not required
- Currently opted-in with Medicare
- Competency managing complex co-morbidities, including CKD, ESRD, and CAD/CV conditions
- Expertise in advanced diabetes management, including CGMs, insulin pumps, and complex insulin regimens
- Experience working with APPs, medical residents, or trainees in a clinical academic setting
- Adherence to evidence-based treatments, behavioral interventions, nutrition, and pharmacotherapy for endocrine disorders and obesity
- Availability for a 32-, 36-, or 40-hour-per-week W-2 role
- Experience providing synchronous telehealth patient care preferred
- Spanish medical fluency desired but not required
- Commitment to patient-centered, culturally competent care
- Excellent communication and interpersonal skills
