Job Description
Virtual Care Provider delivering CBT to patients diagnosed with cancer at Daymark Health. Collaborating with multidisciplinary team to address mental health needs and create care plans.
Responsibilities:
- Provide virtual care primarily via CBT to patients diagnosed with cancer
- Conduct initial needs assessments for patients
- Collaborate with Health Partners, Nurses, and Nurse Practitioners
- Monitor side effects and changes in patient’s function or behavior
- Consult with licensed clinical team members for complex issues
- Document patient interactions in EHR system
- Integrate collaborative care programming into Daymark care model
Requirements:
- Bachelor's degree and Master of Social Work (MSW) or MSW-equivalent program
- Active Licensed Clinical Social Work (LCSW) license
- At least two years of clinical work experience
- Adept at using CBT or similar techniques
- Working knowledge of medical terminology
- Licensed (or willing to be licensed) in Rhode Island, Massachusetts, and Connecticut
- Comfortable using Slack, Zoom, Google Suite, and EHR systems
- Ability to establish rapport with diverse populations
- Organized, communicative, and professional in writing
