Job Description

Remote LPN guiding chronic-care patients through HealthSnap’s remote monitoring platform. Educating patients, reviewing health data, and coordinating care-plan support.

Responsibilities:

  • Complete phone consultations with patients enrolled in care management and/or remote patient monitoring programs
  • Provide support and education about patients’ chronic conditions
  • Educate patients about health conditions and empower them with lifestyle and behavior strategies
  • Assist patients in setting and reaching goals aligned with provider-approved care plans
  • Maintain accurate and up-to-date patient records and document interactions and care plans per protocol
  • Address patient concerns and barriers to care, finding practical solutions to improve adherence and outcomes
  • Communicate clearly, compassionately, and effectively with patients
  • Follow approved workflows for communicating patient needs to providers
  • Participate in training sessions, team meetings, and quality improvement initiatives
  • Respond to remotely transmitted patient data, including blood pressure, blood glucose, weight, and pulse oximetry, according to approved partner workflows
  • Manage an assigned group of patients throughout the month and assist with additional patients or patient tasks as assigned

Requirements:

  • Current, valid, and in good standing Multistate/Compact Nursing License (LPN/LVN)
  • Additional state licenses may be required
  • 3+ years of experience in primary care practice, cardiology, internal medicine, home care, or chronic care management/remote patient monitoring
  • Strong communication and interpersonal skills
  • Excellent organizational and time management abilities
  • Proficiency in using electronic health records (EHR) and care management software
  • Ability to work independently and as part of a team
  • Empathy and a patient-centered approach to care
  • Reliable internet connection
  • HIPAA-compliant work area
  • Proficiency with virtual communication tools such as Zoom and Slack
  • Ability to work Monday through Friday, 9:00 am to 5:30 p.m. Eastern Time, unless otherwise specified
