Job Description
Remote LPN guiding chronic-care patients through HealthSnap’s remote monitoring platform. Educating patients, reviewing health data, and coordinating care-plan support.
Responsibilities:
- Complete phone consultations with patients enrolled in care management and/or remote patient monitoring programs
- Provide support and education about patients’ chronic conditions
- Educate patients about health conditions and empower them with lifestyle and behavior strategies
- Assist patients in setting and reaching goals aligned with provider-approved care plans
- Maintain accurate and up-to-date patient records and document interactions and care plans per protocol
- Address patient concerns and barriers to care, finding practical solutions to improve adherence and outcomes
- Communicate clearly, compassionately, and effectively with patients
- Follow approved workflows for communicating patient needs to providers
- Participate in training sessions, team meetings, and quality improvement initiatives
- Respond to remotely transmitted patient data, including blood pressure, blood glucose, weight, and pulse oximetry, according to approved partner workflows
- Manage an assigned group of patients throughout the month and assist with additional patients or patient tasks as assigned
Requirements:
- Current, valid, and in good standing Multistate/Compact Nursing License (LPN/LVN)
- Additional state licenses may be required
- 3+ years of experience in primary care practice, cardiology, internal medicine, home care, or chronic care management/remote patient monitoring
- Strong communication and interpersonal skills
- Excellent organizational and time management abilities
- Proficiency in using electronic health records (EHR) and care management software
- Ability to work independently and as part of a team
- Empathy and a patient-centered approach to care
- Reliable internet connection
- HIPAA-compliant work area
- Proficiency with virtual communication tools such as Zoom and Slack
- Ability to work Monday through Friday, 9:00 am to 5:30 p.m. Eastern Time, unless otherwise specified
