Job Description
Care Navigator supporting New York patients with chronic conditions through HealthSnap’s remote monitoring platform. Providing education, phone consultations, documentation, and response to transmitted health data.
Responsibilities:
- Complete phone consultations with patients enrolled in care management and/or remote patient monitoring programs
- Provide support and education about patients' chronic conditions
- Educate patients about health conditions and empower them with lifestyle and behavior strategies
- Assist patients in setting and reaching goals aligned with provider-approved care plans
- Maintain accurate and up-to-date patient records and document interactions and care plans according to protocol
- Address patient concerns and barriers to care and find practical solutions to improve adherence and outcomes
- Communicate clearly, compassionately, and effectively with patients
- Follow approved workflows for communicating patient needs to providers
- Participate in training sessions, team meetings, and quality improvement initiatives
- Respond to remotely transmitted patient data, including blood pressure, blood glucose, weight, and pulse oximetry, according to approved partner workflows
- Establish relationships with assigned patients to empower chronic illness management and improve health outcomes
Requirements:
- Current, valid, and in good standing New York Nursing License (LPN/LVN) required
- Additional state licenses may be required
- 3+ years of experience in primary care practice, cardiology, internal medicine, home care, or chronic care management/remote patient monitoring
- Strong communication and interpersonal skills
- Excellent organizational and time management abilities
- Proficiency in using electronic health records (EHR) and care management software
- Ability to work independently and as part of a team
- Empathy and a patient-centered approach to care
- Reliable internet connection
- HIPAA-compliant work area
- Proficiency with virtual communication tools such as Zoom and Slack
- Additional Compact Nursing License preferred
