Job Description

Remote LVN coordinating chronic-care plans and member services for Alignment Health, a senior-focused healthcare company. Supporting patients, families, providers, and case managers telephonically.

Responsibilities:

  • Manage and coordinate health care for members with short-term chronic care needs within the scope of licensure
  • Implement, coordinate, and monitor provider-designed care plans across the care continuum
  • Coordinate and monitor member progress and services for compliant, cost-effective care
  • Ensure member access to services appropriate to their health needs
  • Identify, assess, and manage members according to established criteria
  • Review and implement care plans and provide education materials to members
  • Coordinate care with Primary Care Physicians, Nurse Practitioners, RN case managers, and specialists
  • Assist providers and case managers with problem-solving and service issues
  • Measure intervention effectiveness to determine case management referrals and outcomes
  • Participate in developing utilization/care management policies and procedures
  • Counsel patients and families on available care options
  • Provide follow-up and implement care plans based on Nurse Practitioner direction
  • Perform duties mostly telephonically
  • No supervisory responsibilities

Requirements:

  • 5 years of clinical case management experience, or any combination of education and experience providing an equivalent background
  • Current, unrestricted LVN license in the state for which you are applying
  • Associate degree preferred
  • Ability to perform each essential duty satisfactorily
  • Ability to meet the physical demands of the role, including frequently lifting and/or moving up to 10 pounds
