Job Description
Nurse Practitioner delivering virtual neurology and memory care through telehealth for Local Infusion patients. Managing assessments, treatment plans, and ongoing clinical documentation remotely in Maine.
Responsibilities:
- Provide direct clinical care to patients with neurological conditions, memory disorders, and/or dementia via telehealth
- Conduct patient assessments
- Develop care plans
- Manage ongoing treatment in collaboration with supervising physicians
- Partner with neurologists and the broader care team to ensure continuity of care
- Educate patients and families on diagnoses, treatment plans, and disease progression
- Maintain accurate, timely clinical documentation in the EMR consistent with regulatory requirements
Requirements:
- Must live in Maine and be licensed in Maine
- Active, unrestricted Nurse Practitioner license in Maine
- Certification in geriatrics, adult-gerontology, or family practice preferred
- 2+ years of clinical experience in neurology, memory care, and/or dementia care settings
- Strong clinical judgment and clinical documentation skills
- Comfortable managing a full patient panel independently with physician collaboration
- Excellent communication skills with patients facing cognitive decline and their caregivers
