Job Description
Neurology Nurse Practitioner delivering virtual specialty infusion care for Local Infusion patients. Managing telehealth assessments, treatment plans, and continuity of care with neurologists.
Responsibilities:
- Provide direct clinical care to patients with neurological conditions, memory disorders, and/or dementia via telehealth
- Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians
- Partner with neurologists and the broader care team to ensure continuity of care
- Educate patients and families on diagnoses, treatment plans, and disease progression
- Maintain accurate, timely clinical documentation in the EMR consistent with regulatory requirements
- Deliver care to Local Infusion patients and partner with physicians and the broader care team to increase patient access and reduce time to treatment
Requirements:
- Must live in New Hampshire and work remotely from NH
- Active, unrestricted NP license in New Hampshire
- Certification in geriatrics, adult-gerontology, or family practice preferred
- 2+ years of clinical experience in neurology, memory care, and/or dementia care settings
- Strong clinical judgement and clinical documentation skills
- Comfortable managing a full patient panel independently with physician collaboration
- Excellent communication skills with patients facing cognitive decline and their caregivers
