Job Description
Nurse Practitioner delivering virtual neurology and memory care for Local Infusion’s specialty infusion patients. Providing telehealth assessments, treatment planning, and ongoing clinical support.
Responsibilities:
- Provide direct clinical care to patients with neurological conditions, memory disorders, and/or dementia via telehealth
- Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians
- Partner with neurologists and the broader care team to ensure continuity of care
- Educate patients and families on diagnoses, treatment plans, and disease progression
- Maintain accurate, timely clinical documentation in the EMR consistent with regulatory requirements
Requirements:
- Must live in New Jersey and work remotely from New Jersey
- Active, unrestricted Nurse Practitioner license in New Jersey
- 2+ years of clinical experience in neurology, memory care, and/or dementia care settings
- Strong clinical judgement and clinical documentation skills
- Comfortable managing a full patient panel independently with physician collaboration
- Excellent communication skills with patients facing cognitive decline and their caregivers
- Certification in geriatrics, adult-gerontology, or family practice preferred
