Job Description
Virtual Nurse Practitioner providing primary and acute care for complex Medicaid patients through CareBridge’s home- and community-based services. Developing treatment plans and coordinating interdisciplinary care.
Responsibilities:
- Provide primary and acute care through telephone and tele-video modalities
- Develop and implement clinical plans of care for adult patients with chronic and complex conditions
- Gather patient histories, physical examinations, and diagnostics as needed
- Develop and implement treatment plans based on patient goals and current conditions
- Identify and close gaps in care
- Address patients’ and families’ physical and psychosocial needs with interdisciplinary team support
- Educate patients and families about medications, side effects, illness progression, diet, nutrition, adherence, and crisis prevention
- Coordinate care with company physicians, other physicians, medical providers, patients, families, and clinical team members
- Maintain patient medical records and documentation in accordance with regulations and company policy
- Participate in continuing education
- Prescribe medication as permitted by state authority
- Collaborate to develop complex care plans aligned with patient health status, goals, and values
- Provide clinical and non-clinical support to patients receiving home and community-based services
Requirements:
- MS in Nursing
- Active, national NP certification
- Valid, current, active, and unrestricted Family or Adult Nurse Practitioner license in Virginia
- Valid, current, active RN Compact license
- 2+ years of experience managing complex care cases
- Experience working with Electronic Medical Records (EMR)
- Experienced Nurse Practitioner candidates
- DEA registration or eligibility preferred
- Active Medicaid number in Virginia highly preferred
- Experience managing complex care cases for developmental disabilities and chronically ill patients strongly preferred
- Must reside within a reasonable commuting distance of the posting location(s), unless an accommodation is granted
- Ability to work Monday–Friday, 8:00 am–5:00 pm EST or CST, with rotating on-call
