Job Description

Nurse Supervisor leading Care Navigators delivering remote chronic-care management and patient monitoring. Improving outcomes through HealthSnap’s proactive digital healthcare platform.

Responsibilities:

  • Manage a team of Care Navigators conducting chronic care management and/or remote patient monitoring programs
  • Create engaged Care Navigator teams that build trusting therapeutic relationships with patients
  • Oversee patient-facing work and lead performance optimization of clinical outreach with selected practices
  • Develop supportive relationships with Care Navigators and equip them with tools, skills, resources, and confidence
  • Retain top talent and foster a positive work environment
  • Promote adherence to partner Workflow Strategy Guides
  • Develop and provide partner-specific workflow training
  • Collaborate with the Clinical Operations Manager and Customer Success Team
  • Assign patient caseloads and tasks to Care Navigators
  • Assist with program enrollment and unenrollment
  • Monitor Care Navigator performance and provide coaching and support
  • Shadow and coach Care Navigators regularly
  • Complete monthly one-on-one meetings and identify workflow gaps
  • Conduct formal coaching sessions, including Corrective Action Forms and Performance Improvement Plans
  • Complete monthly Quality Assurance reviews of calls and documentation
  • Promote adherence to policies, procedures, legal, and regulatory requirements
  • Plan and lead weekly team meetings

Requirements:

  • 5+ years of experience in primary or specialty care practice, home care, or chronic care management (preferred)
  • Supervisor and/or lead experience, preferably in a remote setting (preferred)
  • Experience in remote monitoring or virtual care management (preferred)
  • LPN/LVN or greater certification (required)
  • Track record of building highly motivated engaged teams, preferably in the remote setting
  • High computer literacy, comfortable learning and using digital health portal and video calls
  • Strong communication and interpersonal skills
  • Excellent time management and organizational skills
  • Engaging personality with the desire to coach and mentor staff
