Job Description
Obesity medicine physician providing remote, evidence-based chronic weight-management care for Form Health’s telemedicine clinic. Collaborating with APPs and dietitians while managing complex patient treatment.
Responsibilities:
- Provide direct patient care focusing on evidence-based obesity treatment and chronic weight management
- Review charts and conduct patient visits to ensure quality and continuity of care
- Provide treatment planning and medical management support for complex cases or escalated concerns
- Manage a patient panel and/or collaborate with APPs and Registered Dietitians specializing in obesity treatment
- Collaborate with and supervise APPs as needed
- Prescribe non-controlled obesity medications
- Ensure clinical documentation, protocols, and treatment pathways are followed consistently
- Participate in team-based care initiatives
- Attend relevant departmental meetings
- Contribute to quality improvement efforts within the obesity medicine program
Requirements:
- Specialized in Obesity Medicine
- Valid medical license to practice and prescribe legally in any US state
- Eligibility to obtain additional state licenses as needed
- American Board of Obesity Medicine (ABOM) certification
- MD or DO degree
- Valid and current Board Certification in Obesity Medicine by ABOM
- Demonstrated clinical experience in obesity medicine and pharmacotherapy, including older generation oral and newer injectable medications
- Active and unrestricted Medical State License in any US State
- Minimum of 2 years as an attending physician, post-residency, in Internal Medicine, Family Medicine or Obesity Medicine, OR completion of an accredited Fellowship program in Obesity Medicine in lieu of experience
- Interest in mentoring, teaching, and collaborating with APPs and Registered Dietitians specializing in obesity medicine
- Demonstrated experience working with APPs, medical residents, or trainees in a clinical academic setting
- Adherence to evidence-based treatments, behavioral interventions, nutrition, and pharmacotherapy for endocrine disorders and obesity
- Must be currently opted-in with Medicare
- Commitment to patient-centered, culturally competent care
- Excellent communication and interpersonal skills
- Open to working one of the following W2 roles: 32, 36, or 40 hours per week
- Synchronous telehealth experience preferred
- Medical fluency in Spanish desired but not required
