Job Description

Obesity medicine physician providing remote, evidence-based chronic weight-management care for Form Health’s telemedicine clinic. Collaborating with APPs and dietitians while managing complex patient treatment.

Responsibilities:

  • Provide direct patient care focusing on evidence-based obesity treatment and chronic weight management
  • Review charts and conduct patient visits to ensure quality and continuity of care
  • Provide treatment planning and medical management support for complex cases or escalated concerns
  • Manage a patient panel and/or collaborate with APPs and Registered Dietitians specializing in obesity treatment
  • Collaborate with and supervise APPs as needed
  • Prescribe non-controlled obesity medications
  • Ensure clinical documentation, protocols, and treatment pathways are followed consistently
  • Participate in team-based care initiatives
  • Attend relevant departmental meetings
  • Contribute to quality improvement efforts within the obesity medicine program

Requirements:

  • Specialized in Obesity Medicine
  • Valid medical license to practice and prescribe legally in any US state
  • Eligibility to obtain additional state licenses as needed
  • American Board of Obesity Medicine (ABOM) certification
  • MD or DO degree
  • Valid and current Board Certification in Obesity Medicine by ABOM
  • Demonstrated clinical experience in obesity medicine and pharmacotherapy, including older generation oral and newer injectable medications
  • Active and unrestricted Medical State License in any US State
  • Minimum of 2 years as an attending physician, post-residency, in Internal Medicine, Family Medicine or Obesity Medicine, OR completion of an accredited Fellowship program in Obesity Medicine in lieu of experience
  • Interest in mentoring, teaching, and collaborating with APPs and Registered Dietitians specializing in obesity medicine
  • Demonstrated experience working with APPs, medical residents, or trainees in a clinical academic setting
  • Adherence to evidence-based treatments, behavioral interventions, nutrition, and pharmacotherapy for endocrine disorders and obesity
  • Must be currently opted-in with Medicare
  • Commitment to patient-centered, culturally competent care
  • Excellent communication and interpersonal skills
  • Open to working one of the following W2 roles: 32, 36, or 40 hours per week
  • Synchronous telehealth experience preferred
  • Medical fluency in Spanish desired but not required
