Job Description
Obesity Medicine Physician delivering virtual evidence-based weight-management care for Form Health’s telemedicine clinic. Collaborating with APPs and dietitians on multidisciplinary patient treatment.
Responsibilities:
- Provide direct patient care focused on evidence-based obesity treatment and chronic weight management
- Review charts and conduct patient visits to ensure quality and continuity of care
- Provide treatment planning and medical management support for complex cases or escalated concerns
- Manage an individual patient panel and/or collaborate with APPs and Registered Dietitians specializing in obesity treatment
- Collaborate with and supervise APPs as needed
- Prescribe non-controlled obesity medications
- Ensure clinical documentation, protocols, and treatment pathways are followed consistently
- Participate in team-based care initiatives
- Attend departmental meetings and contribute to quality improvement efforts within the obesity medicine program
Requirements:
- MD or DO degree
- Valid and current Board Certification in Obesity Medicine by the American Board of Obesity Medicine (ABOM)
- Demonstrated clinical experience in obesity medicine and pharmacotherapy, including older generation oral and newer injectable medications
- Active and unrestricted medical state license in any US state, with eligibility to obtain additional state licenses; IMLC preferred but not required
- Minimum of 2 years as an attending physician, post-residency, in Internal Medicine, Family Medicine, or Obesity Medicine, OR completion of an accredited Fellowship program in Obesity Medicine in lieu of years of experience
- Interest in mentoring, teaching, and collaborating with APPs and Registered Dietitians specializing in obesity medicine
- Demonstrated experience working with APPs, medical residents, or trainees in a clinical academic setting
- Adherence to evidence-based treatments, behavioral interventions, nutrition, and pharmacotherapy for endocrine disorders and obesity
- Open to working a W-2 role of 32, 36, or 40 hours per week
- Currently opted-in with Medicare
- Commitment to patient-centered, culturally competent care
- Excellent communication and interpersonal skills
- Synchronous telehealth patient-care experience preferred
- Bilingual candidates with medical fluency in Spanish desired but not required
