Job Description
Physical Therapist delivering skilled in-home care for Premier Home Health Providers patients. Evaluating mobility, creating treatment plans, and coordinating recovery services across California communities.
Responsibilities:
- Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans
- Deliver physical therapy services in accordance with the physician’s plan of care and California’s Physical Therapy Practice Act
- Educate patients and their families or caregivers on therapeutic exercises, equipment use, and recovery goals
- Collaborate with physicians and the care team, report changes in patient condition, and participate in case conferences
- Supervise Physical Therapy Assistants and, when PT is the sole skilled service, oversee Home Health Aide care bi-weekly
- Manage documentation including clinical progress summaries and discharge planning
- Identify needs for additional home health services and coordinate referrals as appropriate
- Perform other duties as required for patient services
Requirements:
- Bachelor’s or Master’s degree in Physical Therapy from an accredited program
- Active California Physical Therapy license
- Minimum 2 years of PT experience; home health background is a plus
- Strong communication skills and the ability to connect with patients and families
- Current CPR certification with AHA
- Valid driver’s license and reliable, insured vehicle
