Lakeland Care, Inc. (LCI) is a Wisconsin-based managed care organization (MCO) providing long-term care services and supports to eligible frail elders and individuals with physical and intellectual/developmental disabilities through Wisconsin’s Family Care program.

Lakeland Care offers the Family Care Program in the following counties: Brown, Calumet, Door, Florence, Fond du Lac, Forest, Kewaunee, Langlade, Lincoln, Manitowoc, Marathon, Marinette, Menominee, Oconto, Oneida, Outagamie, Portage, Shawano, Vilas, Waupaca, Winnebago, and Wood.
