Job Description
Care Manager coordinating chronic and advanced primary care for Curana Health’s senior patients. Reviewing records, closing care gaps, and supporting patients, caregivers, and providers remotely.
Responsibilities:
- Provide comprehensive care management services for patients enrolled in Advanced Primary Care Management, Chronic Care Management, and other value-based care programs
- Review electronic health records for long-term care nursing facility patients to identify gaps in care
- Review and approve initial and ongoing health questionnaires completed by care management team members
- Conduct scheduled monthly telephone outreach to patients and/or caregivers
- Develop patient-centered care plans
- Educate patients and their durable medical power of attorney on APCM or CCM benefits
- Support quality gap closure through clinical discovery
- Facilitate orders, referrals, and prior authorizations through the virtual care support team
- Escalate abnormal diagnostic test results to Curana providers
- Collaborate with providers to deliver coordinated, patient-centered care
- Communicate patient health updates to Curana providers
- Communicate treatment plans and health updates to caregivers effectively and caringly
- Serve as the primary liaison between providers and the administrative support team
- Perform other duties as assigned
Requirements:
- Must hold an active, unrestricted Compact LPN license
- Ability to obtain additional state licenses, as needed
- 2+ years of experience in nursing required (24 months minimum); care settings may include inpatient, outpatient, or skilled nursing facilities
- Knowledge of pathophysiology and accepted treatment protocols for common health diagnoses, including diabetes, chronic heart failure, and chronic obstructive pulmonary disease
- Ability to analyze patient records to identify gaps in care and report findings to the provider
- Ability to work in a remote environment free of distractions
- Proficient computer skills and ability to adapt to various technology platforms
- Excellent written communication skills
- Demonstrated experience using clinical data to guide decision making
- Ability to function independently and as a member of an interdisciplinary care team
- Prior experience in Chronic Care Management (CCM) and/or care management preferred
- CCM certification strongly preferred
- Experience working with Electronic Health Records preferred
