Job Description

Vynca Lead Care Manager coordinating healthcare, behavioral health, and social services for vulnerable U.S. members. Developing care plans, facilitating access, tracking outcomes, and advocating for client wellness.

Responsibilities:

  • Assess member needs across physical health, mental health, substance use disorder, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and community-based services
  • Oversee development of client care plans and goal setting
  • Offer services through office-based, telehealth, or field-based services where members can access them
  • Connect clients with needed social services and supports
  • Advocate for clients with healthcare professionals
  • Use evidence-based practices such as Motivational Interviewing, Harm Reduction, and Trauma-Informed Care
  • Conduct outreach and engagement to facilitate linkage to the ECM program and log activity in the CRM system
  • Evaluate client progress and update SMART goals
  • Provide mental health promotion
  • Arrange transportation
  • Complete documentation and outcome measures within care-plan timeframes
  • Maintain current patient health records in EMR and other business systems
  • Complete monthly reporting for program compliance
  • Attend assigned training
  • Coordinate person-centered care with healthcare providers, specialists, pharmacists, social service agencies, caregivers, families, and other Care Team members
  • Coordinate healthcare benefits, provide education, and facilitate timely, cost-effective access to care

Requirements:

  • 2+ years of experience as a care manager, care navigator, or community health worker supporting vulnerable populations
  • Willingness and ability to work Monday–Friday, 8:30 a.m.–5:00 p.m. Pacific Time, with potential evening and weekend flexibility
  • Working knowledge of government and community resources related to social determinants of health
  • Clean driving record
  • Valid driver’s license
  • Reliable transportation
  • Excellent oral and written communication skills
  • Positive interpersonal skills
  • General computer skills
  • Working knowledge of Google Workspace, Microsoft Office, and the internet
  • Applicants must reside in Arizona, Colorado, Florida, Georgia, Illinois, Nevada, North Carolina, Oregon, Texas, Utah, or Washington
  • Legal authorization to work in the United States
  • Must complete background screening before employment
  • Patient-, client-, or customer-facing employees must be vaccinated against influenza, subject to accommodation consideration
  • Bilingual English/Spanish preferred
