Job Description
Lead Care Manager coordinating person-centered services for Vynca, which provides comprehensive care for people with complex needs. Managing care plans, referrals, benefits, records, and outcomes remotely.
Responsibilities:
- Assess member needs across physical health, mental health, substance use disorder, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and community-based services
- Oversee development of client care plans and goal setting
- Offer office-based, telehealth, or field-based services where members reside, seek care, or can access services most easily
- Connect clients with needed social services and supports
- Advocate for clients with healthcare professionals
- Use evidence-based practices including Motivational Interviewing, Harm Reduction, and Trauma-Informed Care
- Conduct outreach and engagement to facilitate linkage to the ECM program and log activity in the CRM system
- Evaluate client progress and update SMART goals
- Provide mental health promotion
- Arrange transportation
- Complete documentation and outcome measures within care-plan timeframes
- Maintain current patient health records in EMR and other business systems
- Complete monthly reporting for program compliance
- Attend assigned training
- Coordinate client care with healthcare providers, specialists, pharmacists, social service agencies, caregivers, family support persons, and other Care Team members
- Coordinate healthcare benefits, educate members, and facilitate timely, cost-effective access to care
- Promote wellness, recovery, independence, resilience, and member empowerment
Requirements:
- 2+ years of experience as a care manager, care navigator, or community health worker supporting vulnerable populations
- Willingness and ability to work Monday-Friday 8:30am-5:00pm Pacific Time, with flexibility for potential evenings and weekends
- Working knowledge of government and community resources related to social determinants of health
- Clean driving record, valid driver's license, and reliable transportation
- Excellent oral and written communication skills
- Positive interpersonal skills
- General computer skills and working knowledge of Google Workspace, MS Office, and the internet
- Bilingual English/Spanish preferred
- Applicants must be located in Arizona, Colorado, Florida, Georgia, Illinois, Nevada, North Carolina, Oregon, Texas, Utah, or Washington
- Legal authorization to work in the United States
- Influenza vaccination required for patient-, client-, or customer-facing roles
