Job Description
LPN/LVN serving as primary contact for patients on the RPM365 platform, engaging and educating them in remote healthcare. Assist with device connectivity, monitor data, and ensure adherence to care plans.
Responsibilities:
- Serve as the primary contact for patients on the RPM365 platform.
- Assist with device setup and app connectivity.
- Engage with patients to ensure adherence to care plans.
- Schedule monthly (or more) check-ins and monitor patient data.
- Educate patients through shared resources.
- Notify healthcare providers of critical alerts.
- Attend virtual team meetings for updates.
Requirements:
- Active New York State nursing license
- Knowledge of chronic conditions and related monitoring devices
- Proficiency with technology, computer platforms, and smartphones
- Strong verbal/written communication skills
- Comfortable with patient outreach and engagement
- Highly organized, detail-oriented, and team-focused
- Independently motivated
