Job Description
Nurse Practitioner conducting health risk assessments in residential settings or telehealth for clinical evaluation. Delivering patient-centered care as part of a team of healthcare providers.
Responsibilities:
- Conduct comprehensive in-home health risk assessments to identify all active and chronic disease conditions
- Take history, examine, determine diagnoses
- Provide written documentation of patient visit, per NCQA standards
- Take patient vital signs, as necessary
- Place case management referrals and communicate with PCP as necessary
- Communicate with patients, caregivers, agency nurses, other providers and vendors as necessary to assure proper diagnosis
- Perform all clinical duties while observing OSHA Universal Precautions
- Maintain patient confidentiality
- Attend required meetings and in-services and participate in committees, as requested
- Participate in professional development activities and maintain professional licenses and affiliations
Requirements:
- Active/unrestricted nurse practitioner license to practice in coverage states
- Board certification in one of the following: American Nurses Credentialing Center (ANCC), American Association of Nurse Practitioners (AANP) or National Commission on Certification of Physician Assistants (NCCPA)
- Active BLS Certification
- Current enrollment in Medicare/Medicaid
- Must maintain a valid driver’s license and good driving record
- Outstanding EHR skills
