Job Description
RN case manager delivering home hospice care to Wellstar patients across Georgia. Coordinating interdisciplinary clinical support, symptom management, medications, and end-of-life care.
Responsibilities:
- Provide clinical home hospice care and travel daily to multiple hospice patients in the Wellstar territory
- Assume primary responsibility for terminally ill patients and caregivers using the nursing process
- Coordinate care with physicians, the Hospice Medical Director, and the interdisciplinary team
- Evaluate and coordinate services from clinical care partners, social workers, chaplains, bereavement coordinators, volunteers, pharmacies, DME providers, and contracted personnel
- Provide ongoing patient and family contact through phone calls and visits
- Educate patients and caregivers on disease processes, symptom management, medications, nutrition, and signs of impending death
- Manage medications and assess symptoms, medication effectiveness, disease progression, safety, caregiving adequacy, abuse or neglect, and suicide risk
- Update individualized plans of care and facilitate changes in level of care or transfers to hospice facilities
- Conduct physical assessments, consult physicians, and pronounce patients at death
- Participate in interdisciplinary meetings, family conferences, case studies, performance improvement activities, committees, hiring, orientation, education, peer review, and case study analysis
- Develop and supervise hospice aide personal care plans
- Maintain electronic medical records and medical components of care plans
- Monitor nursing care quality, safety, infection control, customer satisfaction, and complaint resolution
- Serve as a clinical resource for PRN RNs, on-call RNs, LPNs, and CCPs
- Comply with Wellstar policies, standards of work, code of conduct, state and federal licensure guidelines, and JCAHO standards
- Perform other duties as assigned
Requirements:
- Associate's degree in Nursing or Nursing Diploma required; Bachelor's in Nursing preferred
- Current RN license: single-state or RN compact/multistate license
- BLS certification required upon hire
- Georgia driver's license or valid state driver's license
- Private vehicle insurance
- Minimum 2 years acute care or hospice/home care experience preferred
- Hospice RN experience strongly preferred
- Exceptional empathy and service skills
- Excellent assessment and clinical judgment skills
- Knowledge of developmental characteristics and disease processes affecting people of all ages
- Effective written and verbal communication skills
- Effective computer use for patient care documentation
- Personal vehicle required
- CHPN certification preferred upon hire
