Job Description

Per diem triage RN providing telephonic hospice support for Florida patients and families. Managing after-hours calls, clinical assessments, referrals, documentation, and comfort-focused care coordination.

Responsibilities:

  • Provide telephonic support to patients and families
  • Establish and maintain positive relationships with team members, patients/families, and referral resources
  • Triage after-hours phone calls and efficiently meet callers’ needs
  • Provide telephone advice and education for patient needs
  • Handle telephone inquiries related to Chapters Health System’s services in a timely, courteous, and effective manner
  • Promote and explain the benefits of Chapters Health System’s services
  • Gather necessary information and documentation to maintain complete, current, and accurate patient records
  • Enter and track patient referral information in the EMR
  • Reassure patients and families by phone and help resolve their questions
  • Recognize when an in-person visit is needed and coordinate the visit
  • Assist patients and families in achieving desired comfort levels
  • Use appropriate support, expert resources, or personnel to resolve complex situations
  • Document patient/family contacts in the EMR and communicate with the Interdisciplinary Group
  • Report technical issues immediately and resolve them within 30 minutes when possible
  • Report to an IPU or Resource Center if a technical issue cannot be resolved, as directed by leadership
  • Report to a company-designated location during emergencies when required
  • Perform other duties as assigned

Requirements:

  • Current Florida or multi-state license to practice as an RN
  • Ability to apply for or obtain a multi-state RN license within 30 days of hire if not already held
  • Florida RN license, active single-state out-of-state nurse license, or active multistate/compact nurse license
  • Ability to apply for or obtain an RN license to practice in Washington, D.C. within 30 days of hire
  • Minimum of one (1) year of nursing experience
  • Hospice or hospital experience preferred
  • Previous experience working with an EMR/EHR system
  • Ability to effectively communicate in English, verbally and in writing
  • Bilingual Spanish preferred
  • Reliable transportation to meet patients’ needs
  • Excellent critical decision-making and problem-solving skills

  • Ability to use equipment with visual and auditory mechanisms

  • Ability to perform essential job functions and physical requirements, including lifting patients and/or equipment, bending, pushing/pulling, and kneeling, with or without reasonable accommodations
  • Satisfactory completion of competency requirements
