Job Description
Enhanced Case Management Pharmacist at Allied Benefit Systems engaging members and supporting their health journey through clinical expertise and compassionate communication.
Responsibilities:
- Conduct comprehensive clinical assessments to identify member needs and opportunities for improved outcomes
- Collaborate with providers and care teams to develop individualized care plans
- Monitor treatment progress and intervene when gaps in care or compliance are identified
- Establish trusting relationships with members through proactive outreach
- Address barriers to treatment adherence
- Partner with internal teams to enhance member experience and health results
Requirements:
- Doctor of Pharmacy (PharmD) required
- Current license to practice pharmacy required
- Minimum of 3 years of experience as a licensed pharmacist in a clinical or patient-facing role
- Additional certifications related to diabetes care and patient education preferred
