Job Description
Part-time physician conducting in-home health evaluations for Signify Health, part of CVS Health. Reviewing medical histories, performing physical evaluations, and connecting members to appropriate care.
Responsibilities:
- Provide in-home health evaluations in members’ private residences
- Engage one-on-one with members and answer health questions
- Review medications and medical history
- Perform physical evaluations
- Conduct additional diagnostic and preventive services when ordered, including diabetic eye exams, spirometry, and bone density screening
- Travel locally and within licensed states as needed
- Conduct virtual visits as needed
- Provide Signify Health services, including Diagnostic & Preventive Services products
- Build trusted relationships and help improve health outcomes
- Connect members to appropriate care and identify gaps in care
Requirements:
- MD or DO qualification
- Active, unrestricted license(s) in coverage area(s)
- Ability to travel locally and within licensed states as needed
- Ability to conduct virtual visits as needed
- Ability to safely and comfortably perform duties in members’ homes that may contain household pets, secondhand smoke, and airborne allergens or irritants
- Multi-state licenses preferred
- Ability to perform in-home health evaluations, including medication and medical history review and physical evaluation
- Ability to use an iPad and clinical workflow
