Job Description
Population Health Manager at CVS Health driving high-quality, cost-effective care. Collaborating with cross-functional teams to implement Population Health programs and metrics for care improvement.
Responsibilities:
- Support the execution and the ongoing refinement of Population Health programs.
- Work closely with functional leaders (Product, Finance, Data Science) in the central office to assist with strategy and program design.
- Work with regional leaders and Care Teams to ensure effective implementation of key Oak Street clinical and business priorities.
- Lead communication and content development related to key population health metrics, including quality measures, utilization measures, and financial measures.
- Other duties, as assigned.
Requirements:
- 5+ years of applicable experience in consulting / healthcare / population health / working with healthcare providers; or other similar fields
- Ability to build the trust and confidence of a wide range of stakeholders
- Track record of effectively solving difficult problems
- Practical, results-oriented mindset
- Ability to manage multiple priorities while maintaining a positive attitude
- Ability to develop business cases, ROI analysis, and financial analysis
- Someone who connects the dots between teams - a proactive communicator who puts individual projects into broader organizational objectives
- Ability to succinctly summarize and synthesize large amounts of information
- Expert in MS Office, Google Apps, project management software
- Pleasantly persistent approach
- Humility
- Quick learner comfortable with a high level of ambiguity
