Job Description
Nurse Practitioner conducting virtual dementia assessments for Tembo Health, a digital health company improving older-adult care. Developing care plans and supporting Illinois patients through coordinated virtual services.
Responsibilities:
- Conduct virtual assessments of patients with suspected dementia, including baseline cognitive assessments and laboratory testing orders as needed
- Conduct medication reviews and recommend reducing high-risk medications or starting symptom-management medications where indicated
- Develop comprehensive care plans with care navigators
- Coordinate care with care navigators, primary care teams, external providers, and specialists
- Support urgent care and other geriatric practice needs as the practice evolves
- Participate in multidisciplinary huddles and shadow care navigators and the Head of Medical
- Advise care navigators on patients' clinical needs
- Help onboard future colleagues and improve and scale clinical programs
Requirements:
- Family or adult-gerontology nurse practitioner with 1 year minimum experience
- Licensed in Illinois
- Ability to be licensed to practice virtual/outpatient care in any state
- Must not be excluded from participating in any state or federal healthcare programs
- Passion for caring for patients with dementia
- Comfort with learning new technology systems
- Comfortable working from home in an all-remote team
- Experience with electronic medical record systems, including Athena Health
