Job Description

RN coordinating hospice addendums, HOPE/SFV assessments, and regulatory audits for an end-of-life care provider. Supporting documentation compliance, patient-family experience, and Care Team education.

Responsibilities:

  • Manage and complete the Hospice Election Addendums (HEAs) process
  • Review missed Hospice Outcomes and Patient Evaluation (HOPE) assessments and Symptom Follow-Up Visit (SFV) assessments
  • Ensure patients and families receive required HEA information within regulatory timeframes
  • Audit HOPE visit requirements and documentation to support hospice quality and regulatory compliance
  • Collaborate with interdisciplinary team members and admission staff to obtain, document, and communicate accurate, patient-specific information
  • Promote an exceptional patient and family experience throughout the HOPE assessment process
  • Identify and provide educational and collaborative opportunities for the Care Team individually or in groups
  • Oversee Repair & Maintenance, Culinary and Hospitality, and Ohio Department of Health requirements for facilities
  • Collaborate with colleagues while working remotely
  • Work full-time from 8:30 AM to 5:00 PM

Requirements:

  • Registered Nurse (BSN preferred) in the state of Ohio without any board actions
  • Minimum of 2 years professional nursing experience; hospice experience preferred
  • Experience in hospice, quality, regulatory compliance, clinical auditing, or a related healthcare quality function preferred
  • Demonstrated knowledge of hospice regulatory requirements, clinical documentation standards, and quality processes
  • Experience reviewing clinical documentation and identifying missed, incomplete, or non-compliant assessments and follow-up requirements
  • Strong communication, organization, critical-thinking, and collaboration skills, with the ability to work independently and manage multiple priorities
  • Experience working effectively in a remote/virtual environment and collaborating with interdisciplinary team members preferred
  • Valid Driver's license (Safe Driving Record)
  • State minimum auto insurance
