Job Description
RN coordinating hospice addendums, HOPE/SFV assessments, and regulatory audits for an end-of-life care provider. Supporting documentation compliance, patient-family experience, and Care Team education.
Responsibilities:
- Manage and complete the Hospice Election Addendums (HEAs) process
- Review missed Hospice Outcomes and Patient Evaluation (HOPE) assessments and Symptom Follow-Up Visit (SFV) assessments
- Ensure patients and families receive required HEA information within regulatory timeframes
- Audit HOPE visit requirements and documentation to support hospice quality and regulatory compliance
- Collaborate with interdisciplinary team members and admission staff to obtain, document, and communicate accurate, patient-specific information
- Promote an exceptional patient and family experience throughout the HOPE assessment process
- Identify and provide educational and collaborative opportunities for the Care Team individually or in groups
- Oversee Repair & Maintenance, Culinary and Hospitality, and Ohio Department of Health requirements for facilities
- Collaborate with colleagues while working remotely
- Work full-time from 8:30 AM to 5:00 PM
Requirements:
- Registered Nurse (BSN preferred) in the state of Ohio without any board actions
- Minimum of 2 years professional nursing experience; hospice experience preferred
- Experience in hospice, quality, regulatory compliance, clinical auditing, or a related healthcare quality function preferred
- Demonstrated knowledge of hospice regulatory requirements, clinical documentation standards, and quality processes
- Experience reviewing clinical documentation and identifying missed, incomplete, or non-compliant assessments and follow-up requirements
- Strong communication, organization, critical-thinking, and collaboration skills, with the ability to work independently and manage multiple priorities
- Experience working effectively in a remote/virtual environment and collaborating with interdisciplinary team members preferred
- Valid Driver's license (Safe Driving Record)
