Job Description
Senior Care Manager mentoring the care management team and ensuring excellence in care at Peak Health. Engaging in onboarding, documentation audits, and performance monitoring.
Responsibilities:
- Onboarding new employees and mentoring care management team
- Performing documentation audits
- Building clinical team members expertise through feedback and coaching
- Ensuring overall excellence in all areas of care management
- Mentoring new Care Managers and assisting with training
- Assisting with monitoring performance standards and productivity
- Presenting complex ideas and data to stakeholders
Requirements:
- Current Registered Nurse license
- 5 years of healthcare clinical experience
- Bachelor's Degree in Nursing OR Associate of Science in Nursing Degree (ASN)
- Management of Medicare and/or Medicaid populations
- 5 years Care Management experience
