Job Description
Health Coach educating CVS/Aetna Medicare members with chronic conditions through group coaching, care coordination, and resource referrals. Supporting healthier aging and self-management from home.
Responsibilities:
- Educate members on Managing Medicines, Nutrition, Safety, Benefits/Resources, Mindfulness, and Working with Care Team
- Instruct group coaching classes for members with chronic conditions
- Provide information, referrals, assistance accessing resources, health coaching, and supportive care to Medicare members
- Help members participate actively with healthcare providers and Aetna Care Management teams
- Document all member interactions in appropriate member records
- Demonstrate empathy toward members and caregivers
- Troubleshoot unavailable resources and secure support to close member gaps
- Follow program workflows and review members’ personal action plans
- Determine whether additional referrals to Care Management, Behavioral Health, or other services are needed
Requirements:
- 2+ years working as a Health Coach
- Associate’s Degree (REQUIRED)
- Ability to obtain a direct/hardwired internet connection to a modem/router within 7 feet of the computer
- Minimum internet speed of 25 Mbps download and 3 Mbps upload
- WiFi and satellite internet are not permitted
- Quiet, secure, private designated home workspace compliant with CVS Health and HIPAA guidelines
- Must work within the state and city where currently living
- Comfortable setting up and using multiple monitors and navigating multiple applications
- Ability to communicate via email, calendar invites, Teams messaging, and virtual meetings
- Experience with Microsoft Office 365 applications or similar Google Workspace applications
- Ability to log in to secure systems such as VPN and EHR portals
- Ability to manage strong passwords and recognize suspicious emails or links
- Ability to troubleshoot common technical issues independently
- Openness to learning new skills as the workplace evolves
