Job Description

Nurse Practitioner providing remote clinical triage and telehealth care for HarmonyCares’ complex in-home primary care patients. Coordinating urgent interventions, follow-up, and ancillary services with local clinical teams.

Responsibilities:

  • Act as clinical resource for clinical triage teams for patients requiring additional clinical evaluation
  • Evaluate and treat patients via telehealth to address acute needs
  • Liaise between central triage resources and local clinical teams by providing patient updates and coordinating follow-up visits
  • Address time-sensitive medication refills, lab results, other patient results, and task messages
  • Coordinate escalation or Emergency Room evaluation with patients’ primary care providers
  • Assist in service recovery for patients requiring timely care
  • Work closely with the pharmacy team for comprehensive patient care
  • Sign home health, DME, death certificates, and other orders as needed
  • Review quality metrics and pod-level performance, identify improvement areas, and support superior HEDIS performance
  • Educate patients on the Urgent Care Triage program
  • Identify gaps in care and coordinate external resources such as home health, hospice, and ancillary services
  • Collaborate with the RCMO and Medical Director on care improvement opportunities
  • Perform additional duties assigned by the Regional CMO and/or Medical Director

Requirements:

  • Must be a certified Nurse Practitioner with active licenses in coverage states, including Ohio
  • Must have active DEA license
  • Excellent written and verbal communication skill
  • Ability to react decisively and quickly in urgent and emergent situations
  • Demonstrated experience with multi-tasking
  • Strong communication skills (written & spoken)
  • Strong organizational skills and attention to detail
  • Experience with telehealth and coordinating care virtually (preferred)
  • Experience with APRIMA EMR (preferred)
  • Multistate License (MSL) (preferred)
  • 1 year of experience at HarmonyCares Medical Group (preferred)
  • Candidates may be required to complete a background check in the state of Florida
