Job Description
Certified Nurse Practitioner providing remote clinical triage for HarmonyCares, a value-based in-home primary care provider. Evaluating acute needs, coordinating follow-up care, and supporting complex patients virtually.
Responsibilities:
- Act as clinical resource for clinical triage teams for patients requiring additional clinical evaluation
- Evaluate and treat patients via telehealth to address acute needs
- Liaise between central triage resources and local clinical teams, providing patient updates and coordinating follow-up visits
- Address time-sensitive medication refills
- Address time-sensitive lab results and other patient results while coordinating care with local teams
- Interact with primary care providers for patients requiring further escalation or Emergency Room evaluation
- Address time-sensitive task messages requiring additional clinical support
- Assist in service recovery for patients requiring timely care
- Work closely with the pharmacy team for comprehensive patient care
- Sign home health, DME, death certificates, and other orders as needed
- Review quality metrics and pod-level performance, identifying improvement areas for superior HEDIS performance
- Educate patients on the Urgent Care Triage program to reduce hospital readmissions and improve outcomes and satisfaction
- Identify gaps in care and coordinate external resources such as home health, hospice, and ancillary services
- Collaborate with RCMO and Medical Director on care improvement opportunities
- Perform additional duties assigned by the Regional CMO and/or Medical Director related to clinical pod performance and outcomes
Requirements:
- Must be a certified Nurse Practitioner with active licenses in coverage states, INCLUDING VIRGINIA
- Must have active DEA license
- Excellent written and verbal communication skill
- Ability to react decisively and quickly in urgent and emergent situations
- Demonstrated experience with multi-tasking
- Strong communication skills (written & spoken)
- Strong organizational skills and attention to detail
- Experience with APRIMA EMR (preferred)
- Experience with telehealth and coordinating care virtually (preferred)
- Multistate License (MSL) (preferred)
- Background check in the state of Florida may be required as part of the hiring process
