Job Description
Nurse Practitioner delivering health risk assessments in-home or via telehealth for patients in need. Operating under collaborative practice for complex healthcare needs in Cleveland, OH.
Responsibilities:
- Deliver annual risk assessment in a residential setting or telehealth
- Conduct comprehensive in-home health risk assessments
- Takes history and examines patients to determine diagnoses
- Provide written documentation of patient visit
- Takes patient vital signs and places case management referrals
- Communicate with patients, caregivers, and other providers
- Perform all clinical duties while observing OSHA Universal Precautions
- Attend required meetings and participate in professional development activities
Requirements:
- Active/unrestricted nurse practitioner license to practice in coverage states
- Board certification in ANCC, AANP, or NCCPA
- Active BLS Certification
- Current enrollment in Medicare/Medicaid
- Must maintain a valid driver’s license and good driving record
- Outstanding EHR skills
