Job Description

RN Care Advocate assessing, coordinating, and advocating for Carewell members. Delivering virtual nursing support, care plans, and resource connections remotely.

Responsibilities:

  • Conduct phone and video assessments of members' health status, medications, recent utilization, and social determinants of health needs
  • Build member-centered care plans with clear goals and next steps, updating them as needs change
  • Complete medication reviews and flag discrepancies, adherence barriers, or potential interactions for provider follow-up
  • Coordinate with primary care providers, specialists, pharmacies, DME suppliers, and home health agencies to close care gaps
  • Support members through care transitions, including follow-up after hospital or emergency department visits
  • Connect members to community resources for food, transportation, housing, and in-home support
  • Reinforce provider instructions and treatment plans in plain language, using teach-back to confirm understanding
  • Help members prepare for appointments, understand benefits, and advocate for themselves
  • Recognize clinical red flags and escalate urgent concerns to the appropriate provider the same day
  • Document every member interaction accurately and within service-agreement timeframes
  • Follow consent, HIPAA privacy, and documentation requirements on every interaction
  • Drive care-gap closure, resource connection, member-reported experience, and timely, accurate documentation outcomes

Requirements:

  • Active, unrestricted RN license in at least one U.S. state
  • Nurse Licensure Compact (multistate) license strongly preferred
  • Located in the United States
  • 3+ years of RN experience, including care management, case management, transitions of care, home health, or primary care
  • Demonstrated ability to build trust with older adults, including those who are skeptical, confused, or hard to reach
  • Strong clinical judgment and ability to recognize when to escalate
  • Proficient with EHRs, care management platforms, and phone/video communication tools
  • Ability to learn new systems quickly
  • Self-directed ability to manage a member load and schedule independently
  • Own reliable computer, high-speed internet, and private HIPAA-appropriate workspace
  • Case management certification (CCM) or similar is nice to have
  • Experience with Medicare, dual-eligible, or older adult populations is nice to have
  • Telehealth or telephonic nursing experience is nice to have
  • Experience with SDOH screening tools or community resource navigation is nice to have
  • Bilingual ability is nice to have; Spanish preferred
