Job Description

Case Management Coordinator advancing Colorado Medicaid home care authorizations at Abby Care. Coordinating agencies, reauthorizations, appeals, and care continuity across Colorado’s LTHH ecosystem.

Responsibilities:

  • Coordinate with county and independent case managers across concurrent cases
  • Drive timely prior authorization submissions, reauthorizations, and appeals through Case Management Agencies
  • Serve as the primary contact for service authorizations, care plan updates, and waiver transition support
  • Build and maintain relationships with Colorado Case Management Agencies through meetings, case reviews, and joint resolution efforts
  • Collaborate with case managers and patients’ families to develop care plans aligned with clinical needs and Medicaid requirements
  • Act as the expert liaison for complex situations involving multiple waivers, emergency authorizations, and appeals
  • Support authorization operations for Colorado HCBS waiver programs CFC, CES, and SLS
  • Build and manage the reauthorization process with county Case Managers to prevent lapses in care
  • Create training programs on Colorado-specific prior authorization requirements
  • Mentor staff on LTHH waiver nuances
  • Establish performance metrics targeting strong approval rates and sub-10-day processing times

Requirements:

  • 5+ years coordinating with Colorado case managers in Medicaid LTHH settings
  • Documented success facilitating seamless authorizations and care continuity
  • Expertise in relationship building, communication, problem-solving, and stakeholder management
  • Proven ability to partner with county-level and Case Management Agency case managers
  • Deep knowledge of Colorado’s LTHH ecosystem, including HCPF policies, prior authorization operations, case coordination, and ColoradoPAR
  • Successful navigation of Colorado waiver program transitions and appeals
  • Experience collaborating with patient families, clinical teams, and multiple case managers
  • Bachelor’s degree in Healthcare Administration, Business, or equivalent is preferred
  • Colorado Medicaid certification and LTHH authorization training strongly preferred
  • Employment contingent upon successful background check, satisfactory references, and required documentation
  • Must answer whether sponsorship to work in the US will be required now or in the future
