Job Description

Document Review Analyst validating medical records and billing documents for Foundation AI, an AI-native claims and litigation document automation platform. Ensuring accurate data capture, verification, confidentiality, and productivity targets.

Responsibilities:

  • Process, segregate, review, and validate healthcare-related documents according to quality standards and process requirements
  • Review HCFA Bills, Non-HCFA Bills, UB-04 Bills, medical records, and liens
  • Use the AI application to tag, validate, and process healthcare documents
  • Identify and extract relevant information from healthcare documentation
  • Prepare, compile, and organize documents for data capture and validation
  • Log and verify receipt of data
  • Transfer information from scanned images, PDFs, and Microsoft files into standardized formats
  • Perform high-volume data entry and compare data against source documents
  • Review and resolve errors, missing pages, and missing information
  • Maintain filing systems and safeguard confidential customer information
  • Perform regular backups
  • Retrieve data and generate reports and summaries
  • Meet daily and monthly productivity and quality targets

Requirements:

  • 1–3 years of experience in the healthcare industry
  • Familiarity with healthcare documentation and medical records
  • Experience reviewing or processing HCFA Bills, Non-HCFA Bills, UB-04 Bills, medical records, or similar documentation
  • Excellent typing speed with high data accuracy
  • Strong attention to detail and thorough document-review ability
  • Proficiency with software applications and ability to adapt to AI-powered platforms
  • Strong organizational and analytical skills
  • High accuracy when validating information against source documents
  • Understanding of data privacy and confidentiality principles
  • Strong communication skills
  • Bachelor’s degree or equivalent qualification, with a minimum of 12+3 years of formal education
  • Reside in India and have work authorization to work in India
  • Flexibility to work evening or night shifts
