Job Description
Registered Nurse delivering in-home infusion therapy and patient education for AmeriPharma, a growing healthcare company. Assessing patients, developing care plans, documenting interventions, and supporting adherence to TPN, IVIG, and IV biologic treatments.
Responsibilities:
- Conduct in-home infusion therapy visits according to the plan of care
- Prepare clinical/nursing notes and document patient interventions in clinical and non-clinical records
- Act as a clinical resource and collaborate with pharmacy departments
- Maintain knowledge of drug side effects and interactions
- Provide telephone and in-person education on therapy protocols, drug side effects, and home infusion procedures
- Explain therapy plans, medications, and adherence importance to patients
- Serve as liaison among patients, families, and the pharmacy
- Perform comprehensive initial and periodic head-to-toe assessments
- Determine patient needs using assessment data and input from the care team, physician, patient, and family
- Manage patient and family expectations regarding services, outcomes, and discharge goals
- Establish primary and secondary diagnoses
- Develop care plans with interventions, supplies, and required services
- Evaluate patient progress and revise care plans as needed
- Perform other assigned duties
Requirements:
- Graduate of an accredited nursing school with a current RN state license
- 2+ years of experience in home infusion and infusion centers
- 2+ years of experience with IV Immunoglobulins and IV Biologics
- CRNI certification is preferred
- Experience with peripheral IV starts, infusion therapy, and phlebotomy for adult and pediatric patients is preferred
- Effective interpersonal, time management, and organizational skills
- Strong written and verbal communication skills
- Ability to maintain consistently accurate records
- Ability to direct information to the appropriate healthcare provider
- Proficient in computer skills, including word processing and efficient use of the internet and email
- Current CPR and BLS certification from the Red Cross and AHA
- Valid driver’s license
- Reliable transportation and automobile insurance
- Ability to sit/stand, move around patients’ homes, operate a computer, communicate by telephone and texting, and lift up to 20 pounds
