Job Description
Home infusion RN providing in-home therapy, assessments, and education for AmeriPharma patients. Supporting IVIG, biologic, and TPN treatment adherence across Shoreline, Washington.
Responsibilities:
- Conduct in-home infusion therapy visits according to the plan of care
- Prepare clinical/nursing notes and document patient interventions in clinical and non-clinical records
- Act as a clinical resource and collaborate with pharmacy departments
- Maintain knowledge of drug side effects and interactions
- Provide telephone and in-person education on therapy protocols, drug side effects, and home infusion procedures
- Ensure patients understand therapy plans, medications, and adherence requirements
- Serve as liaison between patients, families, and the pharmacy
- Perform comprehensive initial and periodic head-to-toe patient assessments
- Initiate preventative and rehabilitative care
- Determine patient needs using assessment data and input from the agency team, physician, patient, and family
- Manage patient and family expectations regarding services, outcomes, and discharge goals
- Establish primary and secondary diagnoses based on patient assessment and home health care focus
- Develop care plans with skilled interventions, medical supplies, and necessary services
- Evaluate patient progress and revise care plans as needed
- Perform other duties as assigned
Requirements:
- Graduate of an accredited nursing school
- Current RN state license
- 2+ years of experience in home infusion and infusion centers
- 2+ years of experience with IV Immunoglobulins and IV Biologics
- CRNI certification is preferred
- Experience with peripheral IV starts, infusion therapy, and phlebotomy for adult and pediatric patients is preferred
- Effective interpersonal, time management, and organizational skills
- Strong written and verbal communication skills
- Ability to maintain consistently accurate records
- Ability to direct information to the appropriate healthcare provider
- Proficient in computer skills, including word processing, internet, and email
- Current CPR and BLS certification from Red Cross and AHA
- Valid driver's license
- Reliable transportation and automobile insurance
- Ability to operate a computer and office productivity machinery
- Ability to communicate constantly with patients and team via telephone and texting
- Ability to lift up to 20 pounds at times
