Job Description
Intake Care Representative supporting Evernorth specialty pharmacy and infusion patient referrals. Guiding patients, providers, and care teams through accurate healthcare intake and onboarding.
Responsibilities:
- Serve as the primary point of contact for patients and provide timely, professional support throughout the intake process
- Help patients understand next steps, gather required information, and connect them with appropriate care teams
- Handle inbound and outbound communication with patients, providers, hospitals, physician offices, and healthcare facilities regarding referrals and care coordination
- Gather, review, and accurately enter referral information into company systems
- Receive and triage referral packets, medical orders, and supporting documentation
- Coordinate home health services based on patient needs, geographic location, and insurance requirements
- Verify treatment timelines, including first-dose administration and home infusion scheduling needs
- Upload and maintain accurate patient records and documentation in the medical record system
- Partner with Intake Coordinators and nurses to move referrals efficiently through the intake process
- Communicate referral updates and pending items to appropriate team members
- Transfer insurance information accurately to Insurance Verification teams
- Deliver exceptional service while maintaining patient confidentiality
- Support departmental operations through administrative tasks, phone support, and document distribution
- Contribute to a collaborative environment focused on quality, accuracy, and patient-centered care
Requirements:
- High school diploma or GED
- Minimum 1 year of experience in a healthcare, medical office, pharmacy, home health, or related healthcare support environment
- Knowledge of medical terminology
- Strong verbal and written communication skills
- Ability to manage multiple priorities in a fast-paced environment while maintaining attention to detail
- Proficiency with Microsoft Office applications, including Outlook, Word, and Excel
- Strong organizational skills and commitment to providing excellent customer service
- Ability to maintain confidentiality and handle sensitive patient information appropriately
- Preferred: experience supporting referral management, patient intake, home infusion, specialty pharmacy, or care coordination workflows
- Preferred: experience working with electronic medical records or healthcare management systems
- Home internet through cable broadband or fiber optic service with at least 10 Mbps download/5 Mbps upload
