Job Description
CVS Health virtual Nurse Practitioner delivering remote primary and urgent care. Providing telehealth assessment, diagnosis, treatment, counseling, and care coordination.
Responsibilities:
- Deliver holistic, evidence-based patient care through a remote technology platform
- Assess, diagnose, treat, and manage health problems
- Provide health counseling and disposition planning for patients aged 18 months and above
- Provide education and treatment for pregnancy prevention, STI prevention, and safer sex practices
- Evaluate primary care, acute, chronically ill, and transitional care patients
- Provide contraceptive care counseling and medication management
- Educate patients on health maintenance and respond to patient care inquiries
- Document all patient care in the EHR according to MinuteClinic policies and procedures
- Coordinate patient care with internal and external colleagues and the patient-centered medical home
- Work within a patient care team including Providers, Collaborative Physicians, paraprofessionals, Pharmacists, and other healthcare team members
- Work independently, prioritize and solve problems, take initiative, and advocate for patients and the practice
- Work the full-time overnight shift from 9:30p to 6a
Requirements:
- Current license in one of: AK, AZ, CA, HI, IL, MN, MO, MT, NM, OR, UT, WA
- Minimum of two years of medically relevant experience or equivalent
- Completion of a Master’s Degree level Family Nurse Practitioner program
- Current National Board Certification
- State of Employment license to practice in the Advanced Practice Nurse role
- Effective verbal, written, and electronic communication skills
- Outstanding organizational skills and ability to multitask
- Initiative, problem-solving ability, adaptability, and flexibility
- Ability to work without direct supervision and practice autonomously
- Proficiency with information management and technology
- Capacity to collaborate with professional colleagues to provide quality care
- Depending on the market, proficiency in speaking and writing additional languages, including Spanish, may be required
- Broadband connectivity
- Quiet setting with a neutral background for conducting visits
- Ability to uphold patient privacy per CVS Health guidelines
- Ability to view and read a computer screen and other electronic devices
- Must be willing and able to obtain additional licensure if hired
