Job Description

Nurse Practitioner delivering virtual post-discharge care for Galileo’s population health practice. Managing transitions, medications, symptoms, and follow-up for complex Medicare and Medicaid patients.

Responsibilities:

  • Cover care needs for the Transitions of Care program
  • Conduct telephonic outreach and scheduled video/phone visits for recently hospitalized and discharged patients
  • Perform medication reconciliation and symptom screening
  • Order needed follow-up labs, medications, or supplies
  • Build patient trust, explain Galileo's care model, and connect patients to services
  • Coordinate care across hospitals, emergency departments, pharmacies, and community providers
  • Collaborate with the nursing team
  • Meet individual performance targets and contribute to team-level population health goals
  • Provide comprehensive transition-of-care follow-up within a 24/7 medical group using 4-hour shifts

Requirements:

  • Certified Nurse Practitioner in Adult, Family or Geriatrics
  • Active state licensure in PA, with the ability to obtain additional state licensures
  • 5+ years of post-graduate clinical experience in Primary Care or Geriatrics
  • 2 of those years may be substituted by time spent working at a Skilled-Nursing facility or Long Term Care facility
  • Direct experience engaging Medicare and Medicaid populations managing complex, comorbid conditions
  • Motivational interviewing skills
  • Comfort in a digital-first environment, with experience in or a genuine appetite for virtual care delivery
  • Solid tech proficiency
  • Excellent written and verbal communication skills
  • Availability to work 24-36 hours/week
  • Ability to work weekends on a monthly basis
  • Bilingual English and Spanish is a strong bonus, not required
  • Familiarity with Google Suite and Slack is a plus, not required
  • Active licenses in TX, NJ, IL, NY, MA, MN, LA, and/or SC are a bonus
