Job Description

Remote Care Coordinator - LVN/LPN licensed in Illinois overseeing chronic care management. Responsible for patient outreach, education, and collaboration with healthcare providers in a remote setting.

Responsibilities:

  • Conduct monthly Chronic Care Management (CCM) calls to assess patient health status and provide patient education
  • Perform medication reconciliations and identify barriers to care adherence
  • Assist with health risk assessments, remote patient monitoring, and transitional care management activities
  • Accurately document all patient interactions within Electronic Health Record (EHR) systems
  • Collaborate with providers and clinic staff to support individualized patient care plans
  • Connect patients with appropriate community resources and support services
  • Maintain HIPAA compliance and patient confidentiality at all times
  • Independently manage schedules, patient outreach, and productivity goals

Requirements:

  • Active, unencumbered Illinois LVN/LPN license
  • Bilingual in English and Spanish
  • Active Basic Life Support (BLS) certification
  • Strong ability to work independently and manage time effectively
  • Excellent communication and critical thinking skills
  • High proficiency with Electronic Health Records (EHR) systems and Google Suite
  • Dedicated HIPAA-compliant home office space with a private, secure environment and locked door

  • Previous experience with Chronic Care Management (CCM) programs and billing workflows
  • Experience in remote healthcare, telehealth, or virtual patient support environments
