Job Description
Case Manager facilitating high-quality patient care remotely while collaborating with an international team. Ensuring communication and coordination between patients and caregivers effectively.
Responsibilities:
- Serve as the primary liaison between patients and caregivers
- Monitor and maintain care quality by completing VBP (Value-Based Purchasing) measures
- Handle escalated non-clinical inquiries from patients and caregivers
- Maintain accurate and up-to-date documentation for all caregivers and patients
- Work closely with the patient care coordinator on scheduling and operational issues
Requirements:
- Registered Nurse with NY License - USRN NY
- Must be bilingual in Spanish
- Minimum of 3 years of case management or related experience
- Excellent verbal and written communication skills
- Strong interpersonal and customer service skills
- Ability to handle difficult or sensitive situations professionally
- Knowledge of HHA Exchange is highly preferred
