Job Description

Case Manager facilitating high-quality patient care remotely while collaborating with an international team. Ensuring communication and coordination between patients and caregivers effectively.

Responsibilities:

  • Serve as the primary liaison between patients and caregivers
  • Monitor and maintain care quality by completing VBP (Value-Based Purchasing) measures
  • Handle escalated non-clinical inquiries from patients and caregivers
  • Maintain accurate and up-to-date documentation for all caregivers and patients
  • Work closely with the patient care coordinator on scheduling and operational issues

Requirements:

  • Registered Nurse with NY License - USRN NY
  • Must be bilingual in Spanish
  • Minimum of 3 years of case management or related experience
  • Excellent verbal and written communication skills
  • Strong interpersonal and customer service skills
  • Ability to handle difficult or sensitive situations professionally
  • Knowledge of HHA Exchange is highly preferred
