Job Description

Advanced Practice Provider delivering remote chronic cardiometabolic care for Devoted Health members. Managing virtual visits, prescription refills, and centralized diagnostic results.

Responsibilities:

  • Conduct focused and thorough virtual assessments of patients with hypertension, hyperlipidemia, coronary artery disease, and diabetes
  • Formulate diagnoses and individualized treatment plans, including medication management, lifestyle modifications, and monitoring recommendations
  • Manage a prescription refill inbox in accordance with established protocols and guidelines
  • Participate in centralized results management, including review, triage, and follow-up on lab and diagnostic results
  • Leverage live portal chat and AI-assisted clinical tools for prescribing, inbox triage, medication renewal, results management, and patient communication
  • Identify and evaluate risk factors, comorbidities, and treatment contraindications
  • Counsel patients on medication adherence and potential side effects
  • Collaborate with PCPs, specialists, pharmacy, clinical nursing, social work, family members, and caregivers
  • Use the electronic health information system for visits and provide interface feedback
  • Maintain accurate patient medical records in compliance with legal and ethical guidelines
  • Participate in quality improvement initiatives and ongoing professional development
  • Adhere to laws, regulations, industry standards, patient privacy requirements, and telehealth regulations

Requirements:

  • Advanced Practice Provider (NP or PA)
  • Three or more years working in outpatient clinical practice
  • Minimum of 2 years of experience concentrated in primary care or a subspecialty with heavy focus on hypertension and lipid management
  • Experience ideally managing hypertension, hyperlipidemia, and primary and secondary prevention of atherosclerotic cardiovascular disease
  • Nurse Practitioners: active and in-good-standing RN and APRN licenses
  • Physician Assistants: active and in-good-standing Physician Assistant license
  • Role licensure and certification in good standing required
  • Ability to get licensed in requested states within 90 days of hire date
  • Willingness to obtain licenses in additional states as needed
  • Thorough understanding of cardiometabolic condition management, including disease process, treatment modalities, medication management, and lifestyle modification
  • Experience with STARS/HEDIS measures, coding clinical comorbidities, and identifying clinical care gaps
  • Virtual care experience preferred
  • Proficiency in English and Spanish preferred
