Job Description
Nurse Practitioner or Physician Assistant conducting in-home health evaluations for CVS Health’s Signify Health. Reviewing medical histories, performing physical assessments, and bridging gaps in care.
Responsibilities:
- Provide approximately 45-minute in-home health evaluations
- Engage one-on-one with health plan members and answer health questions
- Review medications and medical histories
- Perform physical evaluations
- Perform additional diagnostic tests when ordered by the member’s health plan, including diabetic eye exams and spirometry
- Use an iPad and intuitive clinical workflow for each evaluation
- Provide education and help members understand and manage their health
- Travel locally and within licensed states as needed
- Conduct virtual visits as needed
- Provide Signify Health Diagnostic & Preventive Services products
- Build trusted relationships and help bridge gaps in care
Requirements:
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Multi-state licenses (preferred)
- Able to cover the following primary locations: Otsego County, Albany NY
- Able to work safely and comfortably in members’ homes, including environments with household pets, secondhand smoke and potential airborne allergens or irritants
- Willingness and ability to travel locally and within licensed states, as needed
- Ability to conduct virtual visits as needed
- Ability to use an iPad and clinical workflow for evaluations
