Job Description
Part-time Nurse Practitioner or Physician Assistant providing in-home health evaluations for Signify Health members. Reviewing health histories, performing physical evaluations, and conducting preventive screenings.
Responsibilities:
- Provide in-home health evaluations for Signify Health members
- Conduct approximately 45-minute visits, including medication and medical history reviews and physical evaluations
- Perform additional diagnostic tests when ordered by the person’s health plan, including diabetic eye exams and spirometry
- Use an iPad and clinical workflow during each evaluation
- Answer health questions and help bridge gaps in care
- Build trusted relationships and help improve health outcomes
- Travel locally and within licensed states as needed
- Conduct virtual visits as needed
- Provide Signify Health services, including Diagnostic & Preventive Services such as spirometry, diabetic eye exams, and bone density screening
- Work in members’ private residences
Requirements:
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Multi-state licenses (preferred)
- Able to cover the primary location: Penobscot, ME
- Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other airborne allergens or irritants
- Nurse Practitioner or Physician Assistant qualification
