Job Description
Nebraska-based nurse practitioner or physician assistant conducting Video Visits for CVS Health’s Signify Health. Reviewing member health histories and helping close care gaps remotely.
Responsibilities:
- Conduct approximately 30-minute online Video Visits with Medicare Advantage, Medicaid, and certain commercial plan members
- Review medications and medical history during Video Visits
- Educate members about their health and answer their questions
- Help close gaps in care and connect members with appropriate care
- Provide 40 hours of availability weekly and consistently provide 320+ capacity units per month
- Maintain a strong visit completion rate, consistently pacing 220+ completed visits per month
- Maintain training, affiliation, and compliance with Video Visit requirements
- Participate in regular manager, team, and clinical meetings
- Meet Signify Health operational, clinical, and member experience KPIs and metrics
- Provide telehealth services only in a private space with adequate internet connectivity
Requirements:
- Board-certified nurse practitioner or physician assistant
- Active state licenses in coverage areas
- Maintain board certification
- Able to obtain and maintain multi-state licenses in coverage area, as required
- FNP or AGNP certification preferred for nurse practitioners
- Experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth visit delivery, or a similar format preferred
- 40 hours of availability weekly
- 320+ capacity units per month
- 220+ completed visits per month
- Maintain training, affiliation, and compliance with Video Visit requirements
- Trained and credentialed for all health plan clients and lines of business in primary coverage state(s)
- Must meet remote visit delivery requirements, including broadband connectivity with video and audio enabled
- Must have a quiet setting with a neutral background for conducting visits
- Ability to uphold health plan member privacy per CVS Health guidelines
- Effective verbal, written, and electronic communication skills
- Ability to work without direct supervision and practice autonomously
- Proficiency with information management and technology
- Ability to operate a computer, phone, iPad, and other electronic devices
- Ability to sit, write, speak intelligibly, and hear member responses
